Introduction to Case Management in Eldercare

INTRODUCTION

Case management in Eldercare focuses on the planning, provision and coordination of care to meet the needs of the older persons as well as the connection between the various stakeholders to optimise the assistance rendered. In this course, participants will have an overview of the older population in Singapore and the arising challenges, social policies, resources pertaining to this age group. This course seeks to equip individuals who are new to this sector with fundamental knowledge of case management in practice in eldercare. Through a role-play, individuals will also learn how to apply relevant geriatric theories and clinical skills to meet the needs of the older persons.

COURSE OBJECTIVE

At the end of the training, participants will be able to apply and demonstrate basic case management skills in working with the older persons, their families, and other agencies.

  • Case management in Singapore eldercare context

  • Community resources, policies, laws and regulations

  • Clinical aspect of case management with older persons 

WHO SHOULD ATTEND

This course is suitable for new social workers, case managers and social service practitioners in the eldercare sector and social service professionals who are interested in Eldercare Case Management.  

AWARD

Participants who meet 80% class attendance will be awarded a Certificate of Accomplishment by the Academy of Human Development.

TRAINERS

Ms Stephy Yang is currently a Social Worker with Fei Yue Community Services. Trained in Sociology and Social Work, Stephy Yang has been working in the elder care sector, specialising in case management and counselling. She serves the frail elderly in the community who has multiple health and social care needs under the Community Case Management Service (CCMS) by identifying, coordinating and supporting the key aspects of seniors’ needs in a holistic and client-centred approach.  

Ms Tricia Teo was an auditor before embarking on her journey of becoming a registered social worker. Since her career switch about 4 years ago, she has been serving the elderly in the community through case management and counselling. Tricia's clinical interest is in geriatric social work and palliative care. She believes in the importance of balancing compassion and accountability in her work.
